Categories of Consumer Health Data Collected

Depending on what you choose to share, consumer health data may include:

• •Written reflections about your experiences, capacity, emotional carrying, body communication, restoration, receiving and participation in life
• •Seven calculated dimension scores
• •The condition labels More Available, Variable or More Occupied
• •Information connecting those responses and results with your first name, email address, completion date and private results token

Sources of Consumer Health Data

Consumer health data comes directly from:

• •The answers and written reflections you submit
• •Results calculated from your submitted answers
• •Consent and completion records generated when you submit the Restoration Index

Purposes for Collection and Use

We collect and use this information to:

• •Calculate your Restoration Index reflection
• •Display your seven dimension results
• •Return your written reflections to you
• •Save the reflection for access through a private link
• •Email the private link when you request results delivery
• •Maintain consent and completion records
• •Respond to privacy requests
• •Secure and operate the service

Categories of Consumer Health Data Shared

We do not sell consumer health data or share it for advertising.

Information is processed by service providers as necessary to operate the requested service. Lovable processes participant records through its hosting, database and managed email infrastructure. Cloudflare processes technical website-delivery requests associated with the domain.

If FG Funnels is connected in the future, it may receive first name, email address, consent choices, private token and seven condition labels for registration or separately authorized marketing communications. It will not receive written reflections, individual statement answers or numerical scores.

We have no affiliates receiving consumer health data. We do not disclose consumer health data to advertising networks, data brokers or artificial intelligence services.

Your Rights

Depending on where you live, you may have the right to:

• •Confirm whether we collect, use, share or sell consumer health data about you
• •Access the consumer health data associated with you
• •Receive information about the service providers or other recipients that process your data
• •Withdraw consent from future collection or processing
• •Request deletion of consumer health data
• •Appeal a refusal to act on a request

We do not sell consumer health data.

How to Exercise Your Rights

Email lovey@loveybradley.com with the subject line “Consumer Health Data Request.”

Describe whether you are requesting access, correction, withdrawal of consent or deletion. We may request information reasonably necessary to authenticate your identity and locate your record. You do not need to create an account.

Requests will be handled without undue delay and ordinarily within 30 days. Certain backup deletions may require additional time according to applicable law and service-provider backup schedules.

To appeal a decision, email lovey@loveybradley.com with the subject line “Consumer Health Data Appeal.” We will respond to the appeal within 45 days. If an appeal is denied, you may contact the attorney general or privacy authority in your jurisdiction.

Withdrawing Consent

You may withdraw consent for future collection or processing by contacting lovey@loveybradley.com. Withdrawal does not affect processing that occurred before the request was received.

Because saved information is necessary to maintain your personalized reflection, withdrawing consent may require deletion of the associated participant record and will cause the private results link to stop working.

Deletion

When a verified deletion request is approved, we will delete the associated participant record from active systems and direct applicable processors to honor the request. Information in archived or backup systems may be deleted according to restoration and deletion schedules permitted by applicable law.

Retention and Private Links

Consumer health data and its associated private results link are retained for 12 months after completion of the Restoration Index. The active participant record is then deleted, and the private link will no longer display the reflection.

Changes to This Notice

We will update this notice before collecting or using additional categories of consumer health data for materially different purposes. Where required, we will obtain affirmative consent before the new collection, use or disclosure begins.
